    Hello all,

    I have used the template for the C.N.I.
    and adjusted it a little, so to complete by hand, in the notary's office, on the day.

    Does this look OK?

    You can preprint the whole thing. The only bit you need to complete by hand in the notaries office is your signature and his.Then he stamps it with his official stamp.

    I agree with David. Once you have an appointment you can also include the date beforehand.

    Personally I'd get rid of the: to marry - for use in China (CNI)
    It's obvious you have prepaired a document for marriage as your declaring your divorced, now single status.
    As for use in China, that's why you have it legalised by the Chinese Embassy. It couldn't be used in any other country than China after legalisation.
